Providers not listed in the scheduling pop-up can be emailed directly to ask about waitlist options.

New Medication Clients:

These appointments CANNOT be scheduled online.

Use the ‘Contact’ form tab or email hello@affinitypsych.com to request intake documents.

Once all documents are completed, we will schedule a medication appointment for you within 2 weeks or less.
